• Employee Survey Action Planning Worksheet and Template
  Use this template to report action planning steps to your Director or AVP. Each unit that has its own data is expected to complete the Action Plan sheet by October 15, 2005 and share this information with all staff by November 15, 2005. Action sheets will be assembled centrally in early November, 2005 to create a master plan document.
• We will ask you to use the Tracking Template to update your Director or AVP on progress. During the Spring 2006, you will specifically asked to provide this information centrally, so we can create a central update. However, you are also encouraged to use it to track progress with your initiatives more frequently.
  The Discussion Worksheet was completed already by most of you, and the Brainstorm Worksheet is a helpful, but optional tool.
